Getting Discharged from Hospital

When you are coming to the end of your stay in hospital, you will go through the following process to help ensure that you get back on your feet as quickly and safely as possible:

I need some additional support

Health and Social Care workers will take a joint approach to work out what is right for you.

This is generally offered as a short term service by the Intermediate Care Team (ICT), or the Short Term Assessment and Reablement Service (STARS).

If you have longer term needs, or if your needs are complex, you will be offered an assessment of needs to determine what those needs are and how they might be met by the East Riding of Yorkshire Council.
