The first step to a Personal Budget is to assess your needs & eligibility for social care funding

The assessment starts by meeting a Care Management Worker. They will contact you to arrange to meet with you. You may like someone else to be present, such as a relative, carer or a friend.

During the meeting the Care Management Worker will listen to you, ask you questions and complete a Supported Assessment Questionnaire. This is to help find out about your situation and your needs.

Contact us for more information and to request an assessment.
